Yes, you must see the doctor for renewal of your disability parking placard every five (5) years.

How do I replace my placard that has been lost, stolen or destroyed?

Fill out the applicant (patient) information on the form, sign and submit to the address at the bottom of the application, along with an indication that your previous placard had been lost, stolen or destroyed. We will issue a replacement placard with the same expiration as the original placard. The original will be cancelled for use and will no longer valid. Please note however, there is a maximum replacement limit of one (1) placard for lost, stolen or destroyed placards.

What type of medical personnel can sign for me to obtain a disability parking placard?

A physician, physician assistant, advanced registered nurse practitioner (ARNP), osteopathic physician, chiropractor, podiatrist or optometrist can sign the application.

I have two motor vehicles. How do I get more than one placard?

If you have an old application, write "Request 2" on the application. If you have a newer application, please check the box.

Can I send in a copy of my application or fax in my application?

No, you must send in the original application. Faxed applications are not accepted.

If I do not have a driver license, what would I put in the space for Driver License/ID number on the application?

If you are not licensed, please put your Oklahoma ID Card number in the space if you have an ID card, otherwise leave the space blank. If the application is for a minor child, insert the child's ID Card number if the child has one, otherwise leave the space blank.

How long will it take before I receive my parking placard?

If the application form is completed properly, please allow approximately 20 business days for processing and mailing. If your properly completed application is presented at the Disability Parking Permit desk at the Building 1 of the Department of Public Safety at 3600 Martin Luther King Avenue, Oklahoma City, Oklahoma, your application will be processed immediately.

What if I do not receive my placard within the 20 business days?

To ensure proper delivery, please make sure that you have given us a proper return mailing address that has a mail receptacle to receive your mail. Also, be sure that you have included any apartment numbers, suite numbers or box numbers in the return mailing address. If you have not received your disability parking placard after 20 business days, you are welcome to call the Disability Parking Permit Desk at (405) 425-2693 to check on the status of your application.

What if I ride in the car with someone else or I have a driver take me?

The placard is issued to the person, not to the vehicle. As long as you are riding in the vehicle your disability parking placard is valid.

You may renew your placard six months (6) before it expires.

Why did I get a temporary placard when I have always had 5 year disability parking placards?

We issue according to what physician marks on the application. A change of doctors can also be the reason for the change. Also, if the application is unmarked, a temporary placard is issued so that you may have a placard to use until a 5 year placard is issued if necessary.

Disabled person's parking spaces are designed to ensure that physically challenged individuals have equal opportunity to use various public and private facilities and are not put at a disadvantage by a medical condition.

If you wish to use a disabled person's parking spaces, you must possess a handicapped person placard issued by the Oklahoma Department of Public Safety. At the moment, the DPS issues two types of placards:

• 5 years placards
• Temporary placards for up to six months.

In order to be eligible for a handicapped person placard, you must have one of the following conditions:

1. Cannot walk two hundred (200) feet without stopping to rest
2. Cannot walk without the use of or assistance from a brace, cane, crutch, another person, prosthetic device, wheelchair or other assistant device
3. Is restricted to such an extent that the person's forced (respiratory) expiratory volume for one (1) second, when measured by spirometry, is less than sixty (60) mm/hg on room air at rest, or
4. Must use portable oxygen
5. Has functional limitations which are classified in severity as class iii or class iv according to standards set by the American Heart Association
6. Is severely limited in his or her ability to walk due to an arthritic, neurological, or orthopedic condition
7. Is certified legally blind
8. Is missing one or more limbs which impair mobility.

If you wish to apply for a placard, you must fill out a Handicap Parking Placard application form, have you doctor sign it, and mail it with a one dollar check to the address provided on the form. Processing of your application takes approximately ten business days and, if your application is approved, the DPS mails a certificate to your home address.

You may use a placard in any one vehicle operated by you or in which you are a passenger. The placard must be attached to the rearview mirror of the vehicle and presented to any law enforcement officer upon request.

To qualify for a Physically Disabled License Plate, you must have a current 5-year parking placard from the Department of Public Safety.

All Physically Disabled License Plates are issued for a period on one year. Applications must be submitted to the Oklahoma Tax Commission. Note: the application must be completed in the same name as your placard.

I have a hazmat endorsement but never really carried a hazmat load because I don't really know how to placard correctly and I've heard stories where people get ticketed at weigh stations for having improper hazmat placards. I am thinking about downloading an app called "Hazmat Placard Wizard Mobile" where you simply punch in data from you bill of lading and it will tell you the correct placards to put on. But I'd also like to learn for myself just to double check. How did YOU learn and is there a website you recommend that makes it easy to understand? And have you ever used a hazmat app such as I described?

I also remember learning in orientation that the placards must be placed at least 3 feet away from any sign or logo but I always see trucks having them way less than 3 feet, and sometimes even right next to the logo. I want to know as much as I can about hazmat. And just to make sure, all truck drivers in the U.S. carry the same little orange hazmat book in their truck right? That way I know we're on the same page.

Shippers that deal with High-Value Freight almost never allow you to watch them load it. Often times they don't even want you to know what the product(s) are that they're shipping, and they'll never tell you the exact worth of the load. They simply want you to do the driving and nothing else. Its always a great idea to ask if you can watch them load it and maybe you'll get lucky and they'll let you, but the chances are very slim. If you do get to watch them load the trailer you're looking for:

2: Count the cases and pallets and ensure that the Bill of Lading matches your counts.

3: The trailer is loaded correctly and in a way that will prevent damages to the product.

4: Inspect the quality of the pallets. You don't want a product loaded on a pallet that is falling apart. It could lean during transit and cause major damage.

5: Verify the temperature of the freight (if applicable) and the Seal/Trailer # as well as the destination.

If there's any discrepancies call your Fleet Manager immediately before signing the Bill of Lading. Most companies require a minimum of 6 months before they'll even consider assigning you a High Value load so don't be under the impression that you'll be going through this during week 3.

Mas o Euromilhões está em queda?

Não, agora não. O Euromilhões é uma poll de dez lotarias. Tem havido um trabalho conjunto, com jackpots mais frequentes, com o Milhão, que tem contribuído para estancar a queda e até a recuperação do jogo.

Get a parking placard for people with disabilities

Apply for a parking placard that allows you to use a disabled parking stall.

A parking placard allows people who cannot walk 50 metres (164 feet) to use disabled parking stalls.

The placards are issued to individuals who provide proper proof that they meet the requirements.

• a temporary placard, issued for between 3 and 12 months
• a long-term placard, issued for 5 years
• a permanent placard that can be renewed every 5 years by the client without additional medical certification

You can only have 1 placard for each vehicle registered in your name.

Licence plate option

You can also get a licence plate that contains the official disabled driver logo. An applicant requesting plates must have the vehicle(s) registered with the name of the person with the disability on the vehicle registration.

Eligibility

Parking placards are available to individuals and some organizations who meet the requirements.

Residents of Alberta

If you’re a resident of Alberta and can’t walk more than 50 metres (164 feet), you may be eligible to apply for a parking placard.

People who have sensory impairments, such as deafness and blindness, are not eligible for a parking placard.

Visitors to Alberta

If you are visiting Alberta, you can use your valid (non-expired) parking placard from your home province/territory or country.

If you have forgotten your parking placard at home, you can apply for a temporary red parking placard to use during your stay. These parking placards must be returned to a registry agent before you leave Alberta.

Organizations

Parking placards can be issued to organizations that provide transportation services to persons with disabilities.
The business can only have 1 parking placard for each vehicle registered in the organization’s name.

The application must be submitted within 6 months of the healthcare professional signing it.

Visitors to Alberta

If you are visiting Alberta and didn’t bring your placard with you, go to a registry agent and request a short-term placard that will cover the length of your visit to Alberta. The placard must be returned to a registry agent before you leave Alberta.

Organizations

Step 1. Write a letter

Write a letter on the organization’s letterhead containing the following information:

• the number of placards requested
• the intended use of the parking placards
• the licence plate numbers of the vehicles

The letter must be signed by an authorized officer.

The number of placards an organization gets depends on the number of active vehicle registrations it has. An organization is only entitled to get enough placards for each active vehicle.

Organizations will get a red placard for 1-year terms only.

Step 2. Submit the letter

Proper placard use

A valid parking placard and matching registration certificate can only be used by the person it is issued to.

Drivers

• The driver must ensure that the placard is valid and they have a registration certificate.
• You can use your valid placard and registration certificate in any vehicle that provides you with transportation.
• The driver must obey other parking laws and pay any parking fees.
• Persons issued a disabled licence plate and registration are subject to a yearly registration fee.

Parking

• Your valid placard must be hung from the vehicle’s rear-view mirror with the registration number and expiry date visible from outside the vehicle.
• The matching registration certificate must be in the vehicle and presented on demand by a Peace Officer.
• You must enter or exit the vehicle when parked in a stall marked for persons with disabilities.
• Municipal bylaws govern these parking stalls on both public and private property.
• A stall or parking space signed for persons with disabilities is not for the exclusive use of one person with a disability; it may be used by others with valid placards.

Placard abuse

• Any abuse or misuse of a parking placard and registration certificate may result in the parking placard registration being cancelled, including:
  • altering
  • defacing
  • duplicating
  • tampering with the placard or registration certificate
  • allowing someone other than you to use the placard and registration certificate
• The person with the disability can’t use disabled parking by remaining in the vehicle while the driver or another passenger leaves the vehicle.
• If parking placards are abused, your parking placard and registration may be cancelled and your parking privileges revoked.
• Service Alberta may notify enforcement authorities if your placard is cancelled.

Reporting parking placard abuse

If you see someone using a parking placard that hasn’t been issued to them, you can report the incident to Alberta Registries.
